Health Improvement Framework: an opportunity to tender
NHS Sutton and Merton is looking to commission an appropriately experienced and qualified organisation to deliver a new and innovative programme of behaviour change. The programme, to support Sutton and Merton adult residents to lead a healthy lifestyle, will be universal but have a targeted approach to reduce health inequalities.
As part of the programme the provider will be expected to develop a service that will deliver interventions to support the prevention of disease e.g. increasing physical activity levels, encouraging positive food choices, encouraging appropriate use of alcohol, quitting smoking, promoting good sexual health and positive mental well being.
This programme of behaviour change support will require the successful provider to link to a number of existing services that currently operate within Sutton and Merton, manage the mapping of available activities operating locally, develop new support teams and directly deliver programmes that support behaviour change.
The provider will be required to
- map the activities on offer across Sutton and Merton that provide support to residents who want to lead healthy lifestyles;
- develop a pathway to support residents of Sutton and Merton choosing to adopt healthier lifestyles;
- engage with health professionals and community partners to promote the service in order to gain referrals and opportunistic sign ups;
- directly deliver face-to-face brief interventions, using the principles of motivational interviewing, that support changes in lifestyle resulting in positive health outcomes. This will be the key aspect of the provider’s role;
- signpost onto existing community and specialist services, where appropriate; and
- develop a monitoring and information tool to track and report on participant’s progress and health outcomes achieved.
