Support for Merton residents with long-term health conditions

The Nelson Commissioning Group, part of Sutton and Merton PCT, is running a series of Expert Patient Programmes in Sutton and Merton, starting in Sutton on 11 June at the Salvation Army, Benhill Road.

Expert Patient Programmes are designed for people living with long-term health conditions. They help individuals take more control of their health by learning new skills to manage their condition better – download the leaflet below for more information.
